A. I certify that the information provided in the application is correct, and complete to the best of my knowledge and belief. In completing this form, I understand that if I willfully give false information or withhold any material information it will render me liable to disqualification for appointment to the service of the Financial Crimes Commission or to disciplinary action which may include dismissal, if already in service of FCC.

B. I also understand and accept that the information given in the application form will be used for the purpose of integrity checking and will be divulged to law enforcement agencies and departments/agencies concerned with security, prevention and detection of crime to determine my suitability for appointment.

C. In connection with my application for employment with the Financial Crimes Commission (FCC), I authorize my former employers or any other authority or person to provide any information relating to me to the FCC. I also understand that officers of the Commission may visit my home and may interview my family members, relatives, past and present employers and my personal referees
